 Employee Attitude Survey
An Employee Attitude Survey is another example of 2-way communications.  In this instance, you are not educating employees, they are educating you and Senior Management.
An Employee Attitude Survey is not a difficult, technical task. The 'programming' is not a concern.
Ignoring potential problems with employee morale will not make them disappear.  Eventually, you will need to open 'the can' and begin to sort out the issues that are causing the problems with morale.  If you ask Employees questions via an Attitude Survey they will give answers that may not be entirely cordial.  
Be prepared for some unpleasant responses and a need to execute a followup Action Plan that can begin to resolve problems, real and perceived.
Step 1The most important Step.  Develop your questions & get input and approval from Senior Management.  They must be onboard.
Step 2We create Tables to 'hold' the Questions and the Responses.  This is simple work.
Step 3We create an Introduction page and a login page.  You edit the Introduction page as it's primary purpose is to explain the purpose of the Survey.
The login must be anonymous, otherwise the responses will be invalid. This 'mode' is called 'multiuser' and is a simple setup.
Step 4We create the webpage for the questions.  The link above (Part 2) is an example. Once again, this is simple.
Step 5Announce the Survey and get maximum cooperation from Managers.  If possible, set up PC's for Users .
Step 6Allow employees approximately 1 month for the Survey.  Responses are collected and you are always welcome 'to peek' at the general input.
Step 7Statistics, reports and charts are prepared showing aggregates on each response.  The responses to open ended Questions are gathered separately for your review.
Step 8Review & work with Senior Management to develop an Action Plan.
Steps 1, 5 and 8 are yours to fulfill.
